A NUMBER OF CHC PRACTITIONER ROLES AVAILABLE FOR PLACE BASED TEAMS THROUGHOUT LANCASHIRE & SOUTH CUMBRIA

PLACE BASES INCLUDE: BLACKPOOL, FYLDE & WYRE, MORECAMBE, SOUTH CUMBRIA, LANCASTER, BLACKBURN WITH DARWEN, GREATER PRESTON, CHORLEY & SOUTH RIBBLE, EAST LANCASHIRE & WEST LANCASHIRE

The post holder will work within the Continuing Healthcare Team concerning the commissioning and reviewing arrangements associated with our Child and Adult continuing healthcare funded placements. The post holder will lead on Best Interest decisions and Court of Protection cases as necessary liaising with relevant agencies as required for CHC funded patients. The post holder will prepare cases for IPP and other panels as required to assist in funding decisions.

The post is a mix of agile working with some office based working, as well as visiting people within their own homes & in placements

Currently we are looking for a motivated and experienced RMN, RGN or AHP who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments so CHC and FNC eligibility can be determined.

An exciting opportunity has arisen within our Continuing Health Care Team. Role involves assessing patients for Continuing Care funding (CHC), holding a caseload and reviewing patients as necessary. Good induction and training offered. Friendly Multi-disciplinary Team. The role is varied and involves working closely with Local Authority, Acute Trusts and with providers

You will work within the Mental Capacity Act and gain experience involving Best Interest decisions and Court of Protection work.

Please note that the main headquarters of Lancashire and South Cumbria ICB will move from Chorley House, Leyland to County Hall, Preston in spring/summer 2023.

Service Providers

1. Monitor progress on care to:

  • Inform future commissioning arrangements
  • Assist in identifying future resource implications
1. Ensure informed information is available on care as required to assist the commissioning function
  • Investigate sources and reasons for referrals and to influence future referral patterns
  • Determine compliance with theICBs commissioning policies
  • Supporting patients to ensure that the most effective care is commissioned, of a good quality and in the appropriate location
  • Achieve the most effective use of the ICBs resources
  • Benchmarking NHS and Non NHS provider services ensuring analysis of National Care Standards Reports and other Clinical Governance data
  • Initial visits to potential providers and ongoing attendance at relevant review meetings
  • Provide and receive highly complex, sensitive information. Communicate complex client condition related information to health and Social care professionals/clients and other groups of staff
  • To maintain and update the Case Management on CMS following each patient contact
  • To keep up to date with Department of Health Guidance Circulars and ensure proper implementation of policies and procedures
Other Key Responsibilities
  • Attend panels as appropriate which consider difficult to place clients/high cost placements. Prepare and circulate reports relevant to meeting, including reports for Court of Protection and attendance at hearings where necessary
  • Attend meetings as required
  • Understand the MCA and DOLS ensuring that this is monitored effectively
  • Be responsible for the case management of CHC Funded Clients
  • Provide the first point of contact for internal and external enquiries about individual cases
  • Provide advice on clinical issues relating to individual cases where appropriate
  • Monitor compliance with terms of individual patient/client Service Agreements. This includes awareness of expenditure.
  • Work in collaboration where necessary with the ICBs Complex Cases Team concerning future need/provision of nursing home and domiciliary care
  • Responsibility and involvement in future service provision which impacts on the whole ICB
  • Participate in the improvement of services as directed by the Quality Monitoring Team and line manager
  • Safeguarding is a key responsibility with this role and the post holder will implement the ICB Safeguarding Policy
  • Maintain contemporaneous clinical records in line with Trust clinical records policy and Professional standards for record keeping.
  • Comply with NMC requirements for Revalidation (if nurse) and maintain professional portfolio that demonstrates effective training has been achieved.
  • Comply with all relevant regulations and policies as appropriate including the NMC/Professional Code of Conduct.
Key Working Relationships
  • Maintain constructive relationships with a broad range of internal and external stakeholders.
  • Participate in relevant internal and external working groups, projects, services, and initiatives which may be highly complex, sensitive, political, and contain contentious information with the aim of providing information and analytical advice to the ICB
  • Work with colleagues in the ICB to develop and implement data collection systems that will provide accurate and timely data and information
  • Communicate information, risks, issues and dependencies, including briefings and reports to ICB senior managers, the Executive Team, and a range of internal and external staff.

  • Undertake complex and detailed information analysis of specific projects and tasks requiring high levels of concentration
  • Make decisions concerning packages of care using managerial and/or clinical skills and knowledge
  • Update, maintain, organise, gather and analyse information to predict and meet future ICB and team needs by identifying best professional practice.
  • Monitor and track risks and issues, proactively resolve and escalate as appropriate
  • Contribute to the information management of tasks and projects, taking a lead for specific tasks
  • Provide coordination of and participate in relevant meetings, providing information advice and support
  • Ensure that data collected is analysed, reported by the team as appropriate and monitor the processing of data and information
1. Communication
  • Provide relevant and timely specialist advice and guidance on functional and information matters.
  • Work with other members of the team, other departments in the ICB, and with key stakeholders to investigate the causes of any variance from planned delivery targets and contribute to the implementation of solutions
  • Support the development of internal and external communications where required by regular contact with other teams in the ICB and with external stakeholders
  • Prepare correspondence and reports as required
  • Liaise with Quality Monitoring Team as required providing information as necessary regarding care issues

  • Support and inform the ICB on the targeting of resources, developing, monitoring, implementing, and evaluating the delivery of plans by providing sophisticated, high quality information and analysis

1. Staff Management
  • Provide specialist training, advice and support on own role and responsibilities · Support training and induction of new staff

  • Update, maintain, organise, gather and analyse information to predict and meet future organisational and team needs by identifying best professional practice

  • Carry out timely and accurate information analysis and reporting on agreed areas of portfolio
  • Responsible for the development and maintenance of databases required for regular reports
