CSH provides a salary sacrifice scheme to help with a variety of items and also has a monthly Health & Wellbeing programmeWhat you will be responsible for· Work collaboratively with other local health care professionals/providers to maintain high quality care and promote practice that is child and family focused within the locality.· Facilitate clear communication with families, colleagues and other community professionals enabling planned care to take place effectively· Take a lead role in developing and implementing the seamless transition of care, supporting early hospital discharge and continuity of care for children and their family.· Support Clinical Team Lead in provision of expert and knowledgeable advice and support to colleagues within the service and other professionals.The role will involve, but not limited to you performing the above.You will have the following qualification/experience:· A minimum of 6 months experience of working as a Care Support Worker in a UK hospital environment with either a Care Certificate or NVQ Level 2 or above in Health and Social Care in the last 2 years OR· 12 months experience of working as a Care Support Worker in a UK hospital environment in the last 3 years.· Undertake regular mandatory training**Depending on the Trust, you may require variable additional certification and skills.Who are NHS Professionals?We specialize in putting people in places to care.
