If you are methodical, motivated, and used to working with accuracy and attention to detail in a very fast paced and often challenging environment then the role of Care Navigation Officer Home care could be for you.
Development: No care sourcing experience is necessary as you will be supported along the journey.
We are looking to recruit full-time permanent Care Navigation Officers, Home care. Successful applicants will be working in the Preston office and from home too!
What we are looking for on your application
- Explain how you will use transferable skills to source care effectively for Lancashire residents using relevant IT systems and software during this exciting time as we navigate a new sourcing system.
- Give examples of how you will effectively manage all allocated caseloads showing commitment and enthusiasm for the role
- Evidence how you can maintain excellent communication skills with internal and external customers and colleagues using a variety of methods.
- Discuss processes and procedures that you follow while adhering to strict timescales and how you escalate cases where necessary
- Demonstrate how you maintain timely, accurate and up to date records and information for management and other professionals
- Describe how you will work as part of a busy team to ensure service targets are met
- Give details of how you will work closely with colleagues, managers, and other professionals to deliver an effective and high-quality service
Further information
The service is open 364 days of the year (closed Christmas Day). Successful candidates will participate in a rota to cover working hours between 8am – 6pm Monday to Friday, weekend and bank holiday working is between 9am – 5pm also on a rota basis.
