South Lincolnshire Rural Primary Care Network includes the following practices; Gosberton Medical Centre, Moulton Medical Centre, Sutterton Surgery, Galletly Practice, Abbeyview Surgery, Holbeach Medical Centre, Littlebury Medical Centre, The Suttons Medical Group and The Deepings Practice

Job Description
To support the Primary Care Network to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.
The role will also include training and development of administrators to support improved co-ordination and effective pharmacy administration within general practice, especially with relation to repeat prescribing systems and processes.
Job Responsibilities
- To undertake medicines reconciliation in accordance with practice protocols and to update patients’ medical records / prescribing accordingly.
- To support the achievement of the practice’s prescribing Quality and Outcomes Framework targets.
- Working with the Practices to contribute to the review and development of repeat prescribing protocols / Standard Operating Procedures to improve the efficiency and effectiveness of repeat prescribing systems.
- To provide advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness.
- To participate in medication audits.
- To assist in the appropriate monitoring and management of prescribing budgets and to liaise with the medicines management team at the CCG.
- To prepare evidence based resources and information to support the implementation of rational cost-effective prescribing.
- To support the Practices to provide medication reviews, patient information leaflets, medicine awareness; and to undertake community visits as appropriate.
- To take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement.
- To respond to medication queries from patients and staff in a professional manner referring to the appropriate GP in accordance with practice protocols.
- To advise on the sourcing and safe management of medicines as appropriate.
- Be responsible for the organisation, planning and of own workload to meet set deadlines.

Responsibilities underpinning the role
- To assist the PCN Director(s) and board in setting and realising the PCN vision, mission and business strategy
- To play a role in the delivery of high-quality primary health care services
- Provide support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes, as a key member of the PCN multi-disciplinary team.
- Working under supervision of the core network member practices i.e. the GPs, practice managers and the Business Manager take referrals from the network practices.
- To make and maintain contact with local health and wellbeing service providers and build a network and knowledge of referral routes to and from service providers, keeping a Directory of Service updated.
- Refer people and/or introduce them to appropriate organisations in Lincolnshire and nationally (where appropriate) e.g. voluntary, statutory (local authority) and local NHS organisations.
- To represent the PCN in cross organisation meetings when agreed e.g. locality/CCG meetings
- Co-ordinate training and development of PCN clinical team
- Collate feedback / analysis data on behalf of the PCN to report to the PCN Board and CCG/NHSE as required
Responsibility for administration

- Computer data entry/data allocation and collation, processing and recording information in accordance with practice procedures.
- Initiating contact with and responding to requests from patients, other team members and associated healthcare professionals and providers.

- Any other duties commensurate with this position.
Duties will vary from time to time under the direction of the Partners / Practice Managers dependent on current and evolving practice workload and staffing levels.
Role Delivery Specifics
Patient Care
To provide medication review services to patients in the practice, domiciliary visits and in residential and nursing homes, and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care.
Assist Partners with the appropriate monitoring and management of their prescribing budgets.
To prepare evidence based resources and information to support the medicine management team and all other relevant health professionals in the implementation of rational cost-effective prescribing.
To help plan, develop and support the introduction of new working processes within the practice to optimise the quality of prescribing.
Identify when there is a need for urgent action or for a step-up in care and alert the relevant professional(s).
Relationships & Communications
To work with all leaders of PCN practices to develop positive, trusting and mutually beneficial relationships
To be committed to the vision and values of the PCN and promote these both internally and externally.
Ensure effective communication is maintained within the Network
Maintain a working relationship with local health care providers enable service delivery of mutual benefit
To establish and maintain effective liaison with stakeholders including health, voluntary, social and education resources, attending relevant meetings as necessary.
Organisational
Work with the PCN Director and board for strategic and operational planning processes to support the PCNs objectives
Support the PCN Directors and board with the writing of development plans as required business and Coordinate meetings for PCN working – periodic board meetings and ad hoc project meetings as required
IM&T
Maintain a SystmOne and EMIS templates to ensure accurate and detailed clinical data is entered into the patient record in a standardised way.
To keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required.
Provide comprehensive outcome focused reports detailing the progress of the service
To ensure the integrity of the operational systems
Training/Education
Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development
Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work
Codes of Conduct
The Social Prescriber must comply with the Code of Professional Conduct set by Allied Health South Lincolnshire and any Code of Conduct developed by the PCN.
