Post: Enhanced Care Home Educator-Dietitian
Band 6, Permanent 22.5 hours
Do you have passion in education and community work? We have an exciting opportunity for you to join our Multi-Disciplinary Enhanced Care Home Team (ECHT) under the Dudley Clinical Hub.
We are looking for a HCPC Registered Dietitian who possess an effective communication and negotiation skills with a desire to inspire people at all levels. As an autonomous practitioner, your innovative skills in the delivery of your role providing education and training, as well as clinical output where appropriate within the Care/ Nursing Homes in Dudley.
Based at Brierley Hill Health and Social Care Centre, you will work within a multi-disciplinary team of Registered General Nurses, Mental Health Nurse Practitioners, Dietitian, Occupational Therapist and Physiotherapist.
The post holder will work within the Community Hub providing leadership, education and support within Care Homes, with an expectation of a reduction in unnecessary, unplanned and avoidable admissions to hospital, supporting safe return of residents, improvement in the quality-of-care provision, and competency levels of care staff.
As a member of the ECHT, you will also play an active role under the wider team of Dudley Clinical Hub (DCH) with the overall aim on preventing unnecessary hospital admission for people living in Care/ Nursing Homes in Dudley.
The post holder will provide clinical expertise in the leadership and support of Care Home staff in delivering a service that helps to identify risks and unmet care needs, enabling care staff to provide an environment that demonstrates quality of care and safety within their facility for all residents.
Where required and appropriate, the Post Holder will assist in addressing specific patient needs including reviews, advise, or signposting of the resident/ Care Home with the overall aim of prevention of unnecessary hospital admission or safe return to their Care Homes. You will support the development of excellent standards by means of supervision, education and facilitation and help equip Care Home staff for role expansion.
