About the role
Weald Primary Care Network (PCN) would like to recruit a forward thinking and enthusiastic Care Coordinator to support their practices in expanding services available to patients.
Weald Primary Care Network (PCN) is made up of 10 member GP practices servicing over 50,000 patients and offers PCN services ranging from First Contact Physio, Social Prescribers, Physician Associate and Clinical Pharmacy.
This role will involve a mix of working from practices and some remote working.

We are a collaborative and ambitious GP Federation working with GP practices across Sevenoaks, Weald, Tonbridge, Tunbridge Wells, Malling, and Maidstone to improve General Practice provision in the area.
