We have an exciting opportunity for a Care Coordinator to join our East STARS Team, offering a tailored individualised support plan for people who are experiencing problematic substance use.

We have an exciting opportunity for a Care Coordinator to join our East STARS Team, offering a tailored individualised support plan for people who are experiencing problematic substance use.
- You will be an integral member of one of our integrated treatment team working closely with lead practitioners, clinical staff and other care coordinators.
- You will be responsible for collaboratively completing recovery planning and case management to enable service users to make positive changes across all stages of their recovery journey.
- You will provide targeted support for service users, from initial assessment, into prescribed treatment, to positively exiting our service and into aftercare, ensuring that every recovery journey is tailored to individual goals and priorities.
- You will take a “whole person” holistic approach, ensuring that every service users package of care encompasses all areas of their wellbeing, not just their substance use, to achieve the best possible outcomes for every person.
