Job description

A Community Mental Health Nurse will act as an effective member of the integrated Community Mental Health Rehabilitation & Enablement Service (CMHRES), with responsibility for a defined caseload of service users who have complex mental health and rehabilitation needs. The main aim of the Team is to focus on rehabilitation, engagement, relapse prevention and enablement to optimise the service users ability to live independently
and enjoy a good quality of life.

Rehabilitation, relapse prevention, education and advice on the warning signs of relapse is an integral function of the post. As a specialist service liaison with other agencies and workers in the statutory, voluntary, welfare and housing sectors is essential in order to promote the most efficient use of services for service users through coordinated case work advice and consultancy.

To work effectively within the Multi-Professional team to deliver high quality care. Act as Care Programme Approach (CPA) care co-ordinator for an average of 15 service users. The rational for limiting the number of service users to each worker is to facilitate the intensity of support and input each client requires and utilise a Team approach in terms of a shared
caseload. Maintaining appropriate, accurate written and computerised records is essential to ensure the safety of the service users and staff.

To hold professional responsibility for assessing individual care needs, risk assessments, development, implementation and evaluation of care plans for service users with the most complex rehabilitation needs.

Being an effective communicator, observer and listener with advanced interpersonal skills, and a refined ability to deal sensitively with this client group.

Maintaining confidentiality at all times particularly in relation to consent in relation to sharing information, whilst respecting the individual service users rights and the Trust’s position on ‘common sense confidentially’.

Adhere to the Trust’s, relevant policies, procedures and guidelines.
Ensure good working relationships with service users, carers, family networks, nearest relatives, other professionals and the public.
Demonstrating understanding and use of clinical supervision to reflect upon and modify own and others practice.

Ensuring that own practice remains within legal, ethical and professional parameters and is open to scrutiny from peers and the governing body.
The post holder must hold a current valid first level registration with the Nurses and Midwives Council.

A requirement of the post is to demonstrate advanced skill in providing on going professional therapeutic interventions, advice, education, and information to service users, their carers & family networks.

Provide supervision, mentorship and leadership for junior staff and Student Nurses, Trainees form other professions as appropriate within the Service.
