Job description
The jobs entails offering nursing care, support and monitoring to patients with cardiovascular disease and heart failure It involves delivering cardiac rehabilitation both face to face in classes and home based (heart manuals and/or digital). It involves optimising treatments prescribed following diagnosis and monitoring effects. It involves education and support around lifestyle issues and risk reduction It is expected successful candidate will achieve clinical skills and NMP qualifications. Training in cardiac rehabilitation delivery options eg REACH HF will be provided as, in due course, will be access to an accredited heart failure course.
The job entails independent travel around the Easington/Seaham area with possible need to support other teams across the CDDFT footprint.
