Job description

We are looking for a motivated, enthusiastic individual with appropriate clinical proficiency and experience to join a dynamic multi-disciplinary team which includes specialist nurses, specialist physiotherapists and physiotherapy assistants.

The service operates Mon-Fri (except bank holidays).

The post holder will perform clinical assessments; using a range of skills and expert knowledge for a range of Cardiac patients. The post holder will be responsible for ensuring that the highest standard of clinical care is provided for patients across CHCP CIC. The interview date will be the 16/2/23 or the 22/2/23 at Highlands HC Hull.

To work as part of a multi-disciplinary team delivering Cardiac rehabilitation services across Hull and East Riding of Yorkshire; aimed at improving the health outcomes for patients’ diagnosed with coronary heart disease and other specific cardiac conditions.

The post holder will work closely with the Heart failure specialist nurses, community nursing teams, secondary care providers, General practitioners, Practice nurses and other health professionals to assess, plan, implement and evaluate the rehabilitation needs of patients following a cardiac diagnosis or event.

The post holder will provide professional help, support and education to patients, relatives and carers that will enable them to effectively manage their condition, improve mortality and morbidity and reduce hospital re-admission.
