$17-$18/ hour
Essential Job Functions (With or Without Reasonable Accommodations):
A. Picks up patients at Apple Rehab homes. Provides the assistance required to safely board patients and caregivers.
B. Ensure wheelchairs and passengers are properly secured before starting van. Depending on route, may pick up several patients for a common destination.
C. On arrival at the medical facility, assists patients in exiting the van and reaching the proper offices or treatment areas.
D. Check the van to ensure that no personal items are left behind.
E. Provides first-aid or CPR if required during patient transport. Immediately notifies emergency staff and dispatcher if skilled assistance is required.
F. Unless there are other scheduled stops, the van should not leave the drop off location while waiting for residents.
G. Completes reports at the end of a shift.
H. Immediately reports traffic accidents to facility as needed. Assists injured persons from first-aid kit until these services arrive. Completes company accident reports.
I. Ensures the van has gas.
J. Exhibits professionalism and upholding the company's values/image at all times
K. Always follow all posted traffic laws.
Physical and Sensory Requirements:
1. Ability to read and write in English in a legible and understandable manner.
2. Ability to remain calm in stressful situations and to take appropriate action.
3. Ability to visually assess all aspects of the resident and to verbally communicate with residents, families, staff, referral sources, medical personnel, regulatory agencies, and consultants.
4. Ability to lift, push, pull, and position patients frequently.
5. Ability to perform considerable reaching, stooping, and bending.
6. Hearing and vision abilities within normal range (corrected) to drive safely and to observe and communicate with patients.
7. Regular exposure to injuries and illness.
8. Exposure to a variety of weather conditions.
9. May be required to stay or be called in especially in emergency situations.

A. Committee and Staff Development Functions:
Maintains own professional competence through participation in continuing education programs, seminars, and training programs. Attends and participates in orientation training, educational activities, committee, and staff meetings as well as all mandatory in-services. Coordinates annual in-services to foster staff understanding of admissions’ issues, resident rights, and other matters specific to resident needs.
Fosters employee involvement and input in the quality assurance process and helps the facility meet the company’s commitment to quality assurance and the mission statement. Participates in quality improvement teams and other committees within the facility, in sister facilities or at the corporate office.
Qualifications:
- Attention to detail, good follow-through skills and ability to prioritize multiple tasks.
- Must hold a currently valid driver's license with a Connecticut Passenger Endorsement and have an excellent driving record.
- Must be innovative and able to find creative ways to serve the variety of resident needs and maintain the customer focus.
- Demonstrates cooperation in assisting others and ability to work as a team.
- Patience, tact, enthusiasm and a positive attitude toward the residents, their support networks and the larger community.
- Satisfactory completion of a company training program in passenger assistance techniques, sensitivity training, and map reading, or demonstrated competence based on previous employment experience.
- Satisfactory completion of CPR, first aid, and seizure training.
