Job description
The post holder will provide autonomous clinical skills in the management of patients under the care of the Frailty team, ensuring that safe and effective quality of care is delivered within the resources available. Working as a Senior Frailty Therapist within the Acute Frailty Service, you will facilitate efficient, effective and coordinated assessment and treatment of frail older adults in the acute medical unit and across the Acute Frailty Pathway for inpatients.
You will ensure that safe, high quality clinical services are delivered in line with national policy, Trust priorities, available evidence and patient care needs.You will support programmes of improvement work related to patient harm, patient outcomes, development and management of frailty clinical pathways and to support the delivery of the Trust and directorate clinical governance priorities and associated work programmes.You will ensure appropriate pathways of care and communication via liaison and referral to other agencies and specialist teams via the MDT as required.You will ensure compliance with Professional policies, National and local guidelines and Trust policies, procedures, and protocols.
