A fantastic opportunity has arisen in Hillingdon for highly motivated and enthusiastic Care Co-ordinator (Nurse/ Occupational Therapist/ Social Worker) to join our Community Mental Health Services.
The CMHTs work with adults covering the Hillingdon borough and the site is based in Uxbridge, close to major motorway networks and within walking distance of the Piccadilly and Metropolitan Tube Lines.
We are a service user led service and work with adults that have mental health issues and require support with their physical health needs.
The role of the CCO is to monitor and support adult mental health service users in the community via medication monitoring, risk assessment and psychosocial education underpinned by the Recovery Model of care.
The CMHTs are multidisciplinary teams, comprising of nurses, occupational therapists, psychiatrists, psychologists, social workers, employment specialists and peer support workers. The teams are supported by good administrative systems and offers an excellent working environment.
The teams have good links with local statutory and non-statutory services and work closely with our colleagues at The Riverside Centre at Hillingdon Hospital.
The post is for 22.5 hours a week (fixed term contract of 6 months) and the CMHT working hours are Mon-Friday 9am-5pm. There is also an opportunities for weekend work across our weekend service.
What we are looking for?
1. Post registration experience with significant years of working with adults with mental health problems and a minimum 6 months of working with adults in a community setting.
2. Excellent communication and interpersonal skills, with a sound understanding of current government legislation, demonstrate the ability to work as part of a MDT and provide high quality care with evidence based therapeutic assessments, interventions and completion of accurate and timely documentation.
3. Specialist skills in mental health assessment, risk assessment, safety planning and medication administration.
4. Car owner/driver would be ideal but not essential as it is about finding the right person
