Job description

To undertake a 22-month development programme in Senior Clinical Healthcare Support – Surgical Assessment Unit through work-based learning and by attending regular training sessions. Supervision and support will be provided during all tasks, enabling the apprentice to work within the organization to gain experience, confidence and to deliver effective and safe patient focused support, to the designated team.Successfully completing the training programme will lead to an Advanced Apprenticeship Level 3 qualification.The successful candidate will be responsible for providing a front-line service to the Surgical Division and for delivering a high standard of customer care to a wide range of client groups, including the ward’s multi-disciplinary team, patients, relatives, and their carers. The post holder must accurately and in a timely manner enter such information on the computerised patient information system .The post holder record patient observations and perform diagnostic and report findings through effective handover to Nurses and Doctors. The Post holder is responsible for phlebotomy and cannulation, chasing results, check and report results . The post-holder must undertake effective forward planning, to ensure that the administrative needs of the service are met and must carry out a range of effective coordination between the Emergency Department and the surgical departments to assist in patient flow within the surgical pathways.

To support and promote a proactive approach towards discharge management within the Surgical Division

To co-ordinate with the Ward Manager in supporting the ward staff in initiating early discharges and to assist the team in being proactive about discharges.

To share information with Bed and Site team re potential discharge / transfers.

To support the Ward Manager to communicate with all members of the multidisciplinary team when a concern is raised regarding a patients discharge, escalating to the Discharge Coordinator as necessary.

To provide support to nursing staff in all matters pertaining to discharge planning and managing processes the day before patient discharge

To check the patient’s vital signs and to communicate changes in their condition.

To complete cannulation phlebotomy course to be able to obtain blood tests, blood gases.

Record and obtain urine samples, swabs, ECG and other investigations to be carried out as directed by the doctors, with appropriate labelling and dispatch for analysis.

To ensure urgent tests are sent to the laboratory and results passed onto doctors when available. Legible documentation should be made in patients’ notes for evidence and audit purposes.

To chase and follow up investigations done internally and pass onto the relevant team

To provide a confidential and high standard general administrative support to the ward/unit, particularly supporting the Ward Manager with tasks such as the extraction of clinical information from ambulatory attendances and the translation of clinical terms, diagnostic and procedural.

To be responsible for ensuring that all documentation relating to a patient’s current episode of care, and produced or used on the ward/unit, is accurately filed (according to documented guidelines) and transported to the Health Records department within a reasonable time of discharge.

To ensure that pathology and x-ray results are organised and available for review by the clinical team where required.

To assist the team to arrange transport internally (via the trust’s online portering system) and externally for patients and equipment when the need arises, in line with the trust’s transport policy.

To assist with patient discharge arrangements by organising transport, liaising with other healthcare providers, arranging outpatient appointments and ensuring that relevant paperwork is available, complete and accompanies the patient.

To provide ad hoc cross-cover, as required, during the absence of ward administrators on other wards.
