Job description

Hi, my name is Kerry, and I am the Community Team Manager for our community team within Children, Family and Young People’s services here at NSFT in Norwich.

As Assistant Practitioner you will:

  • Support the registered nurse in the running of a shift
  • Assess service users’ needs in conjunction with a registered professional.
  • Work independently on own initiative to meet the needs of services users.
  • Liaise and refer with external organisations as required promoting partnership and working to ensure holistic care.
  • Ensure service users’ needs are accurately reflected in health record documentation.
  • Assist service users and carers to access information and support.
  • Support, promote and value equality and diversity.
  • Work well both independently and with the multi-disciplinary team
