What you can expect to be doing:
The role will involve undertaking comprehensive assessments focusing on the needs of Clients and Carers, supporting, and enabling people to minimise dependence and support independence through self-directed support, commissioning, monitoring and reviewing packages of care to meet individual assessed needs.
You will be working as part of an integrated multidisciplinary team delivering outcome-based services. You will be expected to demonstrate excellent communications skills, advocating for adults to assist them to achieve their outcomes, including signposting and providing information and advice.
About you:
Being able to relate to a wide range of people with varied needs and social backgrounds is key, as well as having knowledge and understanding of the services available. The role will offer a great chance for you to develop while working including the opportunity to undertake a qualification on an Apprenticeship Programme in Adult Care.
With training and support from your manager and the team you will have responsibility for your own workload of allocated cases, being able to help your clients directly, ensuring their needs are being met in the most effective way.
