Full Time Assessment Specialist (Marin County)
Non-profit social service agency seeks licensed clinicians or certified and experienced substance use disorder counselors to conduct screening and ASAM Criteria-based and other assessments; serve as the primary liaison with community partners and referring entities; coordinate placements into residential and outpatient treatment; create and maintain client charts in compliance with regulatory agencies and funders; and provide coordinated case management with community treatment providers. The Assessment Specialist will also facilitate individual and group counseling services as assigned. This position will provide assessment services in community and jail-based settings.
Under the Supervision of the Program Manager, the Assessment Specialist will be responsible for scheduling and conducting assessments; providing referrals and care coordination; submitting treatment authorization requests to secure residential placements; developing progress notes, treatment plans, and other documentation of services provided and; follow-up case management.
POSITION RESPONSIBILITIES:
Conduct screening and assessments for substance use disorder treatment, via phone and in-person, to determine the medical necessity for treatment.
Make referrals for placement in residential and outpatient substance use disorder treatment and provide follow-up case management services
Maintaining participant files and documentation of services
Coordinating community services
Collecting and analyzing participant data
Participating in Utilization Review Committee
Engaging in ongoing communication with program participants, treatment staff, and program management
ESSENTIAL DUTIES:
Possessing a current Substance Abuse Counseling Certification from a State of California Accredited Institution is necessary;
Possession of a current LMFT or LCSW is preferred;
Demonstrated experience working with substance use disorder populations in a treatment or case management environment is necessary;
Experience conducting substance use disorder treatment assessments, screening and placement into appropriate treatment modalities, ideally utilizing ASAM Placement Criteria.
Experience with substance use disorder treatment models and an understanding of treatment modalities is expected;
Knowledge of community resources and services is preferred;
Must possess a valid driver's license.
