An exciting opportunity has arisen for an Aspiring Nurse Practitioner to join our acute care at home service in the South West and South East. We are looking for a dynamic, highly skilled, nurse to join our team
The practitioner will provide an alternative medical workforce within our service, providing acute care to patients at home. They will work as an autonomous expert, providing acute assessment, intervention within our 2 hour urgent care and develop a medical management plan for that patients on our virtual wards .
Interest is sought from independent Non-Medical Prescribers. You will practice at an advanced level, demonstrating in depth knowledge and competence in all aspects of acute care within the patient's home setting, encompassing enhanced assessment, critical thinking and clinical management skills that are evidence based, whilst delivering high quality patient centred care.
The successful applicant will have University Hospitals of Derby and Burton NHS Foundation Trust (UHDB), Midlands Partnership Foundation Trust (MPFT) and The Royal Wolverhampton NHS Trust (RWT) responsibility to promote clinical excellence in the care of patients referred to them, providing expert professional and independent clinical care which enables the coordination of a multi professional seamless service for all patients referred through the service.
