Aftercare Coordinator
About the role…
You will be working in the Aftercare team with service users who have completed their treatment. The Aftercare service is a 12-week groupwork programme focusing on relapse prevention, motivation and goals and also offers 1-1 sessions and is based around the 5 ways to well-being model.
The role is full time, 37 hours a week based in Forest Hill, Southeast London.
We are looking for enthusiastic, motivated, and reliable individuals, with a willingness to learn.

Who will you be working with…
We work in partnership with other providers in the borough. You will be working directly with a small team in the Aftercare service and more widely with the Primary care recovery service (PCRS).
