Job overview

The neighbourhood based care group are recruiting 2 advanced clinical practitioners (ACP) to facilitate the delivery of high quality holistic, person centred care. This would be across all of our services which include therapies, nursing, specialist teams and primary care.

The ACPs will provide clinical supervision to care group colleagues in a variety of roles, supporting personal and professional development of clinicians. They will be required to lead on patient safety, quality improvement, contribute to the community research agenda and provide clinical mentorship, so that excellence in clinical practice can flourish.

Main duties of the job

The post holders will be required to work consistently within the four pillars of advanced clinical practice. Their clinical leadership will enable and enhance capacity and capability within multi professional teams. Advanced clinical skills in assessment, diagnosis and treatment is required, to support in the management of complex cases within a variety of settings.

Essential criteria
  • Relevant Professional qualification – RGN, RMN, HCP
  • Primary degree
  • Master’s degree in advanced Practice
  • Non-medical prescriber
  • Mentor qualification

  • Community nursing specialist qualification

Knowledge and Expertise

Essential criteria
  • Awareness of current national and local agenda in health and social care
  • Demonstration of high-level knowledge of multi-agency working (i.e., housing, social services, independent sector, etc)
  • Knowledge and understanding of clinical governance
  • Quality improvement and change management
  • Demonstrate the ability to work in a highly demanding environment
  • Teaching and mentorship clinical setting
  • Ability to work independently and make autonomous clinical decision within own area of competence
  • Knowledge of community profiling and health-needs assessment to respond to community needs
  • High level communication skills, both written and verbal
  • Knowledge of audit / research and experience of analysing findings to improve practice
  • Clinical supervisor and ability to reflect on own practice

Experience

  • Extensive post qualification experience
  • Portfolio of evidence showing achievements in practice/speciality

  • Ability to work autonomously at advanced practice level and make expert clinical decisions to support the development of services and inform practice
  • Ability and evidence to lead on related research and audit in relation to the development of advanced practice
