An exciting opportunity has arisen in the HDFT Acute Medicine team to join a growing dynamic team of Advanced Clinical Practitioners.
The post is for a Band 8a qualified, or band 7 trainee, Advanced Clinical Practitioner who has completed, or is in process of completing, a MSc in Advanced Practice. The role will involve being an integral member of the multidisciplinary team, and as part of safe delivery of essential services. The ACP will be required to practice as part of a 24-hour team delivering a 7-day service, when required. The post complies with Agenda for Change terms and conditions.

Utilise expert clinical knowledge and advanced skills to assess, diagnose, plan, implement and initiate evidence based clinical management of patients within a defined area of practice.
Conduct advanced health assessment and clinical examinations of patients with both simple and complex presentations, which require medical care. Make care management decisions based on interpretation of results, ensuring that optimum physical and psychological needs are met and is ethically based.
Undertake and become competent to carry out a wide variety of clinical procedures, which require advanced levels of knowledge and highly developed skills requiring accuracy. The clinical procedures will vary depending on the specific area of practice.
Manage complete episodes of care, and follow up working in partnership with all members of the team. This will involve delegating and referring as appropriate to optimise health outcomes and patient experience.
Demonstrate competence in responding to emergencies utilizing clinical judgment and decision making to initiate appropriate interventions and treatment.
To develop a health promotion and prevention approach to case management with the ability to comprehensively assess patients for risk factors and early signs of ill health.
Participate in MDT meetings, ward rounds, case presentations and other relevant meetings which contribute to the decision making process in the management of the patient.
