Join a great team providing support to local older people when leaving hospital.
Hours: 27.5 hours per week Mon – Fri. Daily working hours can be 9 - 3, 9:30 - 3:30 or 10 - 4. Half hour unpaid break.
Contract: Fixed term until 30th November 2023
Responsible to: Nightingale Service Manager
Location: White House Community Centre, Hampton. Some remote / home working will also be available in line with our Hybrid Working Policy.
Salary: £24 - 27k FTE DOE plus 5% Employer Pension Contribution.
Age UK Richmond upon Thames are a local independent charity providing support for older people throughout the London Borough of Richmond upon Thames. Our wide range of services have a positive and long-lasting impact on the health and wellbeing of local older people.
We currently have an immediate excellent opportunity for flexible and proactive individual to join our Nightingale Home from Hospital service, which provides support to older people over the age of 65 living in the London Borough of Richmond upon Thames on their return home from hospital. We would like the successful candidate to start immediately if possible.
The team is involved in organising and delivering tasks which facilitate a speedy hospital discharge / make the home safer to avoid readmission and avoid admission in the first place. This includes preparing the home for discharge; fitting keysafes; moving furniture; shopping; arranging travel and providing advice.
This busy role will include:
- Taking referrals into the service and adding to the CRM / database.
- Updating CRM / database and gathering information for the team to support clients.
- Working and communicating with other staff, volunteers, clients, family members, social workers, hospitals and other voluntary groups.
- Liaising with Age UK Richmond staff, volunteers and handyperson teams to ensure tasks essential for a safe discharge are complete.
We are looking for an individual with excellent administration and IT skills, and preferably experience in a health or social care related field. A proactive, enthusiastic and highly organised approach is also essential.
